Disconnecting from server or game without any disconnection prompts

Hey guys,

Is anyone else getting disconnected from the game, but the game continues playing until you try to pick something up or go to sleep? It seems like the only way I can tell if it disconnected is if I can't pick up berries/wood or if I try to go to sleep and my character just lays there without the sleep animation sequence starting. It was only every now and then, but it's happened 4 times in the 2-1/2 hours today. I just can't keep going like this and losing progress.

I don't know how to tell if it's my computer, the game, or the server here it's hosted on. I never had this problem before hosting my game on survival servers a couple weeks ago.
